Steps for renewing your passport book or card by DHL. First, you must meet all five eligibility requirements to use the Mail-In Renewal program.
You have your passport in your possession to submit with your application.
Your passport is undamaged other than normal wear and tear.
Your passport was issued when you were age 16 or older.
Your passport was issued within the last 15 years.
Your passport was issued in your current name or you can document your name change.
Items you need
- Social Security Number if you have one
- Most Recent U.S. Passport
- One 5×5 cm Passport Photo that meets requirements
- Credit or Debit Card to make your payment online
Application
Your journey begins at the website for U.S. Department of State, U.S. Passport Applications and Forms. Filling out the DS-82 application form fully and correctly will help to ensure timely processing of your application.Once you login at the site, you will use the Form Filler to select the correct form. However, because the site selects the correct form based on the information you input. It may generate a different form from what you expect. You must complete all pages of the form, print it single-sided, and sign and date it.
Social Security Number: You must provide a Social Security number if you have been issued one. If you do not have a Social Security number, enter all zeros in the DS-82 form and submit a signed and dated statement. Click here for more information.
Most Recent U.S. Passport: You must submit your most recent passport. This passport will be returned to you.
Photo: Provide one 5×5 cm photo that meets all requirements .
Payment: Pay the passport renewal fee online and include a printed copy of your payment confirmation receipt.
Name Change Documents: If the name you currently use is different from the name on your most recent U.S. passport, you must provide the original certified copy of the legal name change document, plus one photocopy. Examples include: marriage certificate, divorce decree, court-ordered name change document. Your name change document will be returned to you.
Ship Your documents: DHL Shipping Instructions will be given to you. Send your completed and signed DS-82 form (printed single-sided), passport, 5×5 cm passport photo, online payment confirmation, name change document (if applicable), and return shipping waybill.
Important Reminder: Please ensure all required documents and forms are included before mailing your application. Incomplete application packets will be returned without processing.
Your new Passport Book or Card will be sent to you using DHL.
